2018 Clerk’s Annual Questionnaire (CAQ)

Here is some useful information about this survey:
Gather your information before entering your responses into the online survey form. You may choose to have different people from your congregation respond to different parts of the survey. Once you have the information, it should take about 30 minutes to complete the survey.

You can use the "Back" button to return to previous pages. The data will not be lost. Your information will be saved as you move from question to question.

Once you have reached the end of the CAQ, you will have an opportunity to review your responses and make changes before you submit the CAQ.

Once you submit the CAQ, an email will be sent to you with a PDF file of your responses. This is the copy of your responses that you are able to save in your files and/or print. The sender will be listed as Deb Worland.

The deadline for submitting your responses to the CAQ is Thursday, November 15, 2018. [Note this submission date is earlier than has been in the past.]

To print a copy of the CAQ before starting the survey, click the following link or copy and paste it into your browser. This link allows you to print the PDF (but you cannot enter your responses on the PDF electronically to us).

After viewing the PDF and gathering your information, go here to complete the CAQ online:

Annual Statistical Report to the General Assembly

also known as Session Annual Statistical Report for 2018

a.  NEW THIS YEAR: THERE IS NO LONGER A WORKBOOK BEING PRODUCED. The program has been updated to include both helpful tools and definitions to complete the process. A PDF has been created related to questions and definitions being asked. It is available below.

b. Go to the Office of the General Assembly’s NEW WEBSITE LINK: It will be active Friday, December 7, 2018. (Do not use www.)

c.   When that page opens, in the middle of your screen under “Statistical Reporting and Frequently Asked Questions," click on Go to the Online Statistics Entry system. You will then be prompted for your User Name and Password.

       d.   Continue entering your stats and submit.

Please plan to submit your Statistical Report by Thursday, January 31, 2019. If you cannot file by this date or have questions about filing the report electronically, please call the Stated Clerk.
